Please include all available treatment records, initial evaluations, chiropractic assessments, diagnoses, clinical findings, examination results, treatment plans, progress notes, and documentation of adjustments performed. I would also appreciate any records related to my cervical spine, lumbar spine, shoulders, and other musculoskeletal conditions.
These records are needed for my personal medical history and to support my Department of Veterans Affairs (VA) disability compensation claim.
